Effective forms management to help healthcare organizations with clinical documentation integrity during COVID-19
Given the urgent need to care for and treat COVID-19 patients, the American Health Information Management Association (AHIMA) has released two query templates to help healthcare organizations with Clinical Documentation Integrity (CDI) processes to save precious time when developing and responding to queries. Quest Diagnostics, the leading provider of diagnostic information services, is providing the templates as electronic forms that can be accessed through Quanum™ Enterprise Content Solutions. The free COVID-19 query templates provide clear guidelines for the types of data required to accurately document the virus, such as symptoms and risk factors, making it easier to meet administrative requirements.
“Quality reporting of health data translates into more effective public health and disease tracking, and Quest is committed to doing all we can to provide better tools,” said Tom Romeo, General Manager, Healthcare IT. “The need for effective data management is elevated during this pandemic, and adoption of well-designed and integrated forms could make the difference in consistently matching the physician to the care provided.”
Clinical documentation is part of every patient encounter, and CDI programs allow providers to review and provide feedback to improve processes that can enhance patient care, clinical outcomes, data quality measures, and accurate reimbursement. A CDI program includes a balance of people, processes, and technology that must work together to ensure success. All of these elements lead to stronger relationships both within and outside an effective health system.
The Center for Medicare and Medicaid Services (CMS) determines the ICD-10 coding guidelines. The new templates are designed to capture high-quality documentation to support the continuum of care. The integrity of clinical documentation is critical during the pandemic because it can impact research efforts to curb COVID-19 and provides government and healthcare leaders with key information for their decision-making processes. The templates are similar to those CDI professionals use to document other diseases.
“I feel immense pride when I see the important work CDI professionals are doing during this pandemic,” said AHIMA CEO Wylecia Wiggs Harris, PhD, CAE. “We believe these clear, concise, and compliant templates will make it easier to document this virus, which in turn gives courageous medical providers more time to focus on patient care.”
